WHY?

The in-office savings plan was designed to make dental care affordable for the whole family. This is not an insurance plan, no claims are filed, and no payments are made to any other healthcare providers.

All patients without insurance are eligible to apply. Participants are eligible upon payment of the enrollment fee, and the plan is in effect for 12 months. It is the responsibility of the patient to utilize the benefits of the plan before the expiration date.

  • Plan runs year-to-date from initial purchase date

  • There will be a $50 reinstatement fee if plan lapses beyond re-enrollment date

  • Cannot be used in conjunction with other dental plans or discounts, including CareCredit

  • NO REFUNDS for premiums will be issued for any reason. It is the participant's responsibility to utilize the plan during the covered dates and no extensions will be given

  • Patient portion of the bill is due on date of service

  • Plans are only honored at Cicero Family Dentistry, even if treatment elsewhere is recommended by our office

  • Membership may be terminated if the member misses multiple appointments
